OSHA 300, also known as form 300, is the Log of Work-Related Injuries and Illnesses. This log is used to both classify and document the severity of a work-related incident. The log must provide details on what happened and how it happened. Employers are responsible for keeping logs for each establishment or site.
For the purposes of this application, TradeTapp is looking for the section pertaining to the OSHA calculation results recorded on the OSHA 300A form. This section is used to calculate a company’s recordable incident rates, lost time rates, and days away rates. Please refer to below screenshot of highlighted information on the OSHA 300A form:

I don't have an OSHA form, but it is still required. How can I continue?
If an prequalification requires an OSHA form or multiple OSHA forms that you do not have, please reach out to the general contractor requesting the prequalification. BuildingConnected cannot change the requirement that the general contractor has set. However, we suggest letting them know the situation and that you can bypass the question by filling any required fields with 0 as the number or N/A for text. If a file is required, you can upload a word document or PDF just stating why you don't have an OSHA form. This way you can complete the application and the general contractor, who will review your responses, is aware that you are entering in arbitrary information for this question.
